How to Nominate

Select the category you wish to nominate for, read the criteria for the selected category in the drop-down links on this page (or nomination_form_2013 to ensure your nominee meets the criteria. Enter the nominees details and your details or the details of the person nominating the Aged Care staff member or volunteer.

If you are not the line manager obtain the line managers support for the nomination. The nominee must be be informed of the nomination and agree to be nominated.

Complete your nomination under the required criteria headings, ensuring you address the criteria within the word limits.

Download a nomination form in pdf format, this also contains the Eligibility Criteria for your nominee. Enter the nominees details and your details or the details of the person nominating the staff member or volunteer.

Before submitting your nomination please ensure, all sections of the form are completed and your nomination address category criteria within the wordlimits. Nominations not meeting the Criteia and word limit will not be included in the judging.

CATEGORY 1 AGED CARE INNOVATIVE PROJECT TEAM AWARD (NEW)

This category provides an opportunity to showcase a collaborative project relating to the care of older people.

The project must show team work and collaboration.

Applications must address the following:

Finalists will be required to present their project at the Awards Presentation Dinner

        1. Project title
        2. Project team members & designations
        3. Purpose/Aims of project
        4. Process, Planning & Implementation
        5. Evaluation/outcomes
        6. Specific benefits for older persons
        7. Evidence of sustainability

Minimum of 500 words and maximum of 1000 words

CATEGORY 2: AGED CARE CULTURALLY SENSITIVE EMPLOYEE OF THE YEAR AWARD

Nominations must show the nominees understanding of the issues and needs of Culturally & Linguistically Diverse (CALD) and Aboriginal & Torres Strait Islander (ATSI) older people.

Criteria Headings:

Provide Examples

        1. Demonstrated Leadership
        2. Demonstrated support of CALD &ATSI Older people

Minimum of 200 words and maximum of 250 words

CATEGORY 3: AGED CARE CARER OF THE YEAR AWARD

This category recognises staff who show professionalism, dedication and excellence in the care of the older person. Nominees must be working with the older person in Residential Aged Care, Community Aged Care, Personal Care, Pastoral Care, Leisure & Lifestyle.

Nomination must provide examples and demonstrate how the nominee is outstanding in their role Nominations must address the above criteria under the following headings:

Minimum of 200 words and maximum of 250 words

CATEGORY 4: AGED CARE SUPPORT PERSON OF THE YEAR AWARD

Nominations for this category include staff who provide support services in one of the following areas; maintenance, administration, catering, cleaning, and gardening.

The nomination must demonstrate why the nominee is outstanding in their area of support.

Minimum of 200 words and maximum of 250 words

CATEGORY 5: AGED CARE VOLUNTEER OF THE YEAR AWARD

This Category is limited to volunteers working directly with older people in Residential or the Community Aged Care in terms of Resident/Client Focus.

Criteria Headings:

Provide Examples

        1. Outstanding Support
        2. Demonstrated understanding of resident/client needs

Minimum of 200 words and maximum of 250 words

CATEGORY 6: AGED CARE HEALTH PROFESSIONAL OF THE YEAR AWARD

This category is open to any health professionals working in Aged Care who are registered with the Australian Health Professionals Regulation Agency (APHRA).

Nominations must demonstrate the nominee’s outstanding performance in terms of leadership and support.

        2. Demonstrated Support

Minimum of 200 words and maximum of 250 words

CATEGORY 7: AGED CARE MANAGER OF THE YEAR AWARD

This category recognises managers who show leadership in their field, (including Nursing, Allied Health, & Support Services) in the care of the older person in Community or Residential Aged Care.

Nominations must demonstrate the nominee’s outstanding performance in terms of Leadership & Support.

        1. Demonstrated outstanding Leadership
        2. Demonstrated outstanding Support

Minimum of 200 words and maximum of 250 words

Online Nomination Checklist and Tips

Time Saving Tips for online nomination form:

  1. Gather all the information you need prior to filling out the nomination form.
  2. Use Notepad, Word or your favorite word processing software and save your nomination essay to your computer in case of computer, browser malfunction or internet connection issue. Use this information to cut and paste in the nomination form.
  3. Every question will need to be answered in order for the form to be submitted. If you miss an entry, the form will ask you to add the information before it can be submitted.

Award Nomination Checklist:

Please complete all sections of the form.

  1. Enter your nomination ensuring you include the nomination category and the nominees name where required, as well as other required information.
  2. Your nomination statement must address the criteria for the nominated category under the criteria headings.
  3. Nominations must be within the category word limits.
  4. Include the nominees supervisor/line manager’s supporting document (no more than 100 words)

Recognising Excellence & Innovation in the Care of the Older Person

The awards aim to recognise staff and volunteers who excel in the provision of care for the older persion

Nomination Form

  • The Nominee Must Accept Nomination
  • Details of person nominating the individual (If not the Nominees Supervisor/Line Manager)
  • Nominations must be approved by the nominees Line Manager to be included in the awards judging.
    Supervisors/Line Managers may be contacted to confirm their support of the nomination.
    The Line Manager/Supervisor is encouraged to provide a statement below to support nominations.
    Please include supporting documentation to the nomination in the next box.
    Supervisor/Line Manager support must be obtained prior to submitting the nomination.
